General Consent for Consultation Services
Nature and Scope of Consultation Services
I understand that consultation services provided by Divergent Insights may be individualized based on the concerns, questions, and needs of the child or family.
Depending on the nature and scope of the consultation, services may include:
- • A detailed intake interview and review of parent/guardian concerns
- • Review of existing educational, psychological, developmental, medical, or other relevant records provided by the parent/guardian
- • Review of IEPs, Section 504 plans, school records, teacher reports, progress-monitoring data, work samples, and other educational information
- • Administration, scoring, and interpretation of appropriate behavior, social-emotional, developmental, adaptive, or other rating scales and questionnaires
- • Gathering information from parents/guardians, teachers, school personnel, or other professionals when appropriate and authorized
- • Direct observation of the child in an educational, home, community, or other relevant setting, when appropriate and agreed upon
- • Consultation regarding learning, attention, executive functioning, behavior, social-emotional functioning, communication, development, or other areas of concern
- • Interpretation and integration of information obtained from multiple sources
- • Discussion of potential areas of need and possible explanations for observed difficulties
- • Recommendations for educational strategies, accommodations, interventions, supports, further assessment, or referrals to other professionals
- • Assistance with understanding existing evaluation results, IEPs, Section 504 plans, eligibility determinations, or school-based recommendations
- • Development of individualized recommendations based on the information gathered
- • Preparation of a written consultation summary or detailed, comprehensive consultation report, when included as part of the agreed-upon service
Consultation Reports
When a written report is included in the consultation service, the report may summarize the referral concerns, relevant background information, records reviewed, observations, rating scale results, other information gathered, professional interpretation, conclusions, and individualized recommendations.
I understand that the content and level of detail of a consultation report will depend on the specific services requested and the information available.
A consultation report is intended to provide professional guidance and may be useful in helping parents/guardians communicate with schools or other professionals and make informed decisions regarding their child’s needs.
Limitations of Consultation
I understand that consultation services may involve multiple sources of information, including interviews, records, rating scales, observations, and written reports; however, consultation is not automatically equivalent to a comprehensive psychological, psychoeducational, developmental, medical, or diagnostic evaluation.
The specific methods, measures, observations, records reviewed, and other services provided will be determined based on the purpose of the consultation and the questions being addressed.
I understand that the conclusions and recommendations provided by Divergent Insights are based on the information available at the time of consultation and may be limited by the scope of the services requested, the availability and quality of information provided, and the settings in which information is gathered.
I understand that a consultation may identify concerns that warrant additional or more comprehensive evaluation by a qualified professional.
Acknowledgment and Consent
By signing below, I acknowledge that I understand and voluntarily consent to consultation services that may include interviews, review of records, behavior or other rating scales, observations, interpretation of information, individualized recommendations, and, when agreed upon, a detailed written consultation report.
I understand the purpose, scope, and limitations of consultation services and have had the opportunity to ask questions before providing consent.
